What is Instrument-Assisted Soft Tissue Therapy (IASTT)?
Instrument-Assisted Soft Tissue Therapy (IASTT) is a hands-on treatment approach that uses specially designed tools—often made from stainless steel—to help identify and treat soft tissue injuries. These tools allow your therapist to apply targeted pressure and friction to muscles, fascia, and other soft tissues in ways that go beyond what hands alone can achieve.

This therapy is especially useful for breaking down scar tissue, releasing tight or restricted areas, and improving movement and function. It’s commonly used when traditional manual therapy isn’t quite enough to address deeper restrictions or chronic soft tissue issues.
How Does IASTT Work?
During a treatment session, your therapist will gently glide the instrument over the skin to feel for areas of tightness, restriction, or tenderness. Once these areas are identified, the tool is used to apply controlled pressure and scraping techniques to stimulate the body’s natural healing response.
This helps to:
- Break down scar tissue and adhesions
- Stimulate blood flow to promote healing
- Restore healthy tissue movement
- Reduce inflammation and relieve pain
Benefits of Instrument-Assisted Soft Tissue Therapy
IASTT offers a variety of benefits for people dealing with chronic pain, injuries, or movement limitations. These include:
- Breakdown of Scar Tissue
- Increased Blood Flow & Circulation
- Improved Range of Motion
- Reduced Muscle Tension and Pain
- Faster Recovery After Injury
Conditions Commonly Treated with IASTT
IASTT is used by healthcare professionals—including physiotherapists, chiropractors, massage therapists, and athletic therapists—to treat a variety of soft tissue injuries and conditions, such as:
- Tendinitis and Tendinopathies (e.g., tennis elbow, Achilles tendinopathy)
- Plantar Fasciitis
- Myofascial Pain Syndromes
- IT Band Syndrome
- Muscle Sprains and Strains
- Post-Surgical Scar Management
Tools You Might See in the Clinic
Some of the most recognized tools in IASTT include:
- Graston® Technique Tools
- Gua Sha Tools (based on traditional Chinese medicine)
While each tool has a different shape and design, they all serve the same purpose—helping your therapist reach deeper layers of tissue and detect problem areas more precisely.
Is IASTT Right for You?
IASTT is often incorporated into a personalized rehabilitation program and can be combined with other therapies such as exercise, manual therapy, and stretching. Whether you’re recovering from an injury, managing chronic pain, or looking to improve athletic performance, IASTT may help accelerate your progress.
